Group Coaching Services Agreement
Evolving Lives Professional Group, LLC
PARTIES
Evolving Lives Professional Group LLC (Coach Alexander Borrello), referred to as "Coach," and the Client, referred to as "Client." Collectively, the Parties agree to the following terms.
TERMS
SERVICES
Services: Grief in Motion Coaching and Support Group through Evolving Lives Professional Group, LLC, facilitated by Coach Alexander Borrello.
Participation Includes:
One hour-long online group meeting per week via Google Meet
Access to a private community forum via the Client Portal on EvolvingLivesInc.com. This forum allows Clients to chat with one another between meetings and receive additional resources from Coach.
PAYMENT
Cost: $20 billed per week (on Mondays) or $60 billed per month (on the 1st of each month).
Payment Methods: A credit card (Visa, MasterCard, or American Express) must be kept on file. Client agrees to update expired cards promptly. Payments are processed via Stripe Inc.
Billing & Cancellation: The Coaching Agreement establishes recurring billing for group coaching services. Client consents to recurring weekly or monthly billing. Client may cancel their subscription at any time via the Client Portal on the EvolvingLivesInc.com website. There are no partial charges nor reimbursement for missed or partially attended sessions on the part of the Client, as Client is purchasing access to the community forum and supplemental materials in addition to the meetings.
MEETINGS, RESCHEDULING, AND REIMBURSEMENTS
Meetings are held online every Tuesday from 6:00 PM to 7:00 PM Eastern Time, excluding weeks where the Tuesday falls on a national holiday or the Sunday-through-Saturday week contains Thanksgiving, Christmas, or New Year's Day. If a meeting must be canceled or the Group Leader is unable to attend, Client will be notified as soon as possible. A replacement meeting will be attempted for another day that same week at the same time; if Client cannot attend the replacement day, or if the week is missed entirely, Client will be reimbursed for the lost meeting. If Client is unable to attend a week due to an emergency or force majeure, Client must notify the Coach (preferably via email at [email protected]) to request consideration for reimbursement. Reasonable requests, as determined by the Coach and Evolving Lives Professional Group, LLC, will be honored.
CLIENT EXPECTATIONS
Attendance: While attendance at weekly meetings is not required, it is advised for optimal growth. Client will not be reimbursed for meetings not attended nor for time missed due to lateness or early departure.
Participation: Client agrees to bring honesty, vulnerability, and openness. Client is always free to decline to share but is encouraged to explore deeply. Client agrees to extend patience and grace to other group members. Part of group members being challenged to explore more deeply, Coach may guide focus toward a specific theme, story, or even Client/member. As Client shall sometimes receive extra time, so shall others. Hearing someone explore their loss more deeply often prompts new thoughts; it is hoped everyone can benefit from this deeper give and take.
Confidentiality: Client agrees to binding confidentiality; details of other participants’ personal challenges cannot be shared outside of the group. This applies to both the video call meetings and to the forum.
Decorum: Client agrees to a reasonable level of social decorum. Members must stay on the topic of grief, respect the lifestyles and opinions of others without judgement, and avoid inappropriate language or controversial non-grief topics. The Group Leader will guide and admonish content in meetings and the forum as necessary.
Action: Client commits to work toward action plans they create and share with the group such that all participants contribute toward community progress.
Repercussions: Client agrees that continued disregarding of these expectations after guidance from Coach may result in having your ability to subscribe and participate revoked by Coach and Evolving Lives Professional Group, LLC.
GRIEF IN MOTION COACHING AND SUPPORT GROUP BASICS
Grief in Motion is a coaching and support group designed to help individuals navigate the complex experience of loss. It is intended for anyone ages 14+ (with parent/guardian permission for participants ages 14-17) who are grieving various types of loss, including suicide, homicide, substance use, military service, sudden illness, or accidents. (Potential group members with other losses may be included/excluded after discussion with and at the discretion of Coach.) Participants will share experiences with fellow group members. All parties agree to mutual confidentiality: no information will be shared outside of the group. Client agrees that no group member can be held liable for recommendations or suggestions provided. Client’s choices are Client’s sole responsibility. As the group is held online, Client agrees to join from a private location and use headphones in public settings to protect others' confidentiality.
Client understands this group is not a substitute for professional advice by legal, medical, financial, or other qualified professionals. While the Group Leader is a grief expert, this service is not medical care or a substitute for counseling, psychotherapy, or mental health care. Client is fully responsible for their physical, mental, and emotional wellbeing. Client agrees to hold harmless Evolving Lives Professional Group LLC and all other group members.
Client understands that this group does not involve the diagnosis or treatment of physical disorders or mental disorders as defined by the American Psychiatric Association. If Client is currently involved in mental health care, they agree to consult with their provider regarding their decision to engage in this group. If mental health issues arise that are not within the scope of this group, the Group Leader will recommend seeking psychotherapy services in conjunction or instead of this group, but the Group Leader is not responsible for Client’s mental or physical health.
Client understands that engaging in this group over the internet involves risks (including, but not limited to: hacking, phishing, and errors in transmission). Client consents to electronic communication and waives any rights against Evolving Lives Professional Group, LLC for damages arising from such transmissions. Please take precautions to protect the confidentiality of electronic communications, including using effective passwords.
CONFIDENTIALITY & MANDATED REPORTING
Under most circumstances, matters discussed remain confidential. However, mandated release of confidential materials is required in situations of suspected child or dependent adult abuse, potential harm to oneself or others, and court-ordered subpoenas.
Regarding Minors: Parents/Guardians have the right to request updates but are requested to respect the minor’s privacy. The Group Leader will notify the signing Parent/Guardian if the minor presents a danger to self or others, exhibits suicide ideation or self-harm, or if there is suspected abuse or neglect.
LIMITATION OF LIABILITY
Evolving Lives Professional Group, LLC's total liability shall be limited to the total amount actually paid by Client. In no event shall Evolving Lives Professional Group, LLC, its coaches, or affiliates be liable for incidental, consequential, indirect, special, or punitive damages. No action arising from the Services may be brought by Client more than one (1) year after the cause of action has arisen.
Disclaimer: Coach employed by Evolving Lives Professional Group, LLC holds a mental health license, but this license is not utilized within the practice of leading this group, and as such Coach is not responsible for any services other than group facilitation, which is not a licensed service. Coach cannot and does not make professionally licensed recommendations. Should Client desire such services, a separate engagement is required.
Guarantees: Coach cannot guarantee results (e.g., remission of distress due to grief). Coach agrees to provide Services in a reasonable and timely manner. Client agrees to take responsibility for their own results.
GENERAL PROVISIONS
Amendment: The Parties may amend this Agreement only by written consent.
Governing Law: Evolving Lives representatives follow the laws and professional regulations of the State of Michigan (USA) and this group will be considered to take place in the State of Michigan (USA).
Notice: Notices shall be effective upon sending via email to Coach at [email protected] or to the Client’s email on file.
TERMINATION
Client signature indicates understanding that Coach may terminate services if Client does not comply with policies or if Client is not benefitting from services.
